Facility Details / Hours of Operation

The GEM program runs from Monday to Friday from 0730-1600.

The program functions as a multidisciplinary team and includes outpatient assessment clinics as well as a day hospital component.

The goals of the program are to increase and optimize independence, function, and quality of life for older adults. The program also aims to assist older adults in achieving the degree of health required to live in the community in order to prevent or delay hospitalization or institutional placement.

Physician Referral

Referrals can also be made through a nurse practitioner.

Referrals can come from an inpatient or outpatient setting.
